Proliance Surgeons has partnered with Datavant to provide release of information services. Datavant will provide Medical Records and Billing Statements. Datavant is unable to provide Imaging. Please contact your Care Center directly for a copy of your Imaging.
How to Request Medical Records

Send requests directly to the Care Center.
By Attorneys
Submit your medical records and billing records requests directly to Datavant. Refer to our Directory to find the appropriate fax number for the Care Center the patient visited.

Send imaging requests directly to the Care Center.
Send balance verifications to [email protected].
By Patients
As one of our patients, you may request a copy of your health information at any time.
How to Request Access to Your Medical Records
- Complete this Online Request Form with direct submission to our release of information vendor, Datavant.
- Your Proliance Care Center may have a patient portal allowing you to obtain a digital copy of your medical records. Please contact the Care Center directly.
